Is divorce effected by obsessive doubt and uncertainty in uttering it, given the absence of desire for it and love for the wife?

Divorce does not occur through self-talk or uttering it under the influence of whisperings (waswasah). One must turn away from these whisperings and not pay attention to them, because indulging them leads to evil and tribulation, and turning away from them is the best cure for them.
