If your wife entered into ihram for 'Umrah, it is not permissible for her to exit from it until after performing the rituals. The sight of blood does not constitute a valid reason for her to abandon her ihram. She is still in her ihram, and she must go to Makkah to perform tawaf, sa'y, and taqsir, and by doing so, she will exit her ihram. She must avoid all prohibitions of ihram. For what she did out of ignorance regarding the prohibitions, there is no sin upon her. However, for acts of damage (such as cutting hair and nails), it is safer to offer a fidyah: fasting for three days, or feeding six poor persons, or sacrificing a sheep. What is obligatory upon her now is to cease all ihram prohibitions and proceed to Makkah to complete her ritual.
