Is it permissible for a person afflicted with continuous flatulence to perform ablution at home before going to the mosque and pray Tahiyat al-Masjid with the same ablution, even if his ablution breaks on the way to the mosque?

Wudu performed at home with the intention of praying Tahiyat al-Masjid (two rak'ahs upon entering the mosque) is not sufficient if one is consistently experiencing flatulence. Instead, wudu for this prayer must be performed upon entering the mosque.
