Is it considered Islamically permissible to dispose of the surplus amount from health insurance deductions, which exceeds what the subscriber and his employer paid?

In commercial insurance, it is permissible to benefit from the amount paid by the company (whether through salary deduction or not), and any excess beyond that must be disposed of in charitable ways for the benefit of Muslims.
