There is no difference between withdrawing cash and using it for purchases; the determining factor is the card's freedom from these two prohibitions. If one or both of them exist, then it is not permissible to use it, and the cash refund for returned goods takes the same ruling. If the bank takes a usurious interest on withdrawing money, then it is not permissible to deal with it.
