Can a wife request a Khula' without her husband's consent, and what are its reasons?

A woman is permitted to seek Khul' from her husband if she dislikes his bad manners, such as harshness and anger, or if she dislikes his physical appearance, such as a defect or ugliness, or if he is deficient in religious practice by abandoning prayer or committing forbidden acts, or if he withholds her rights to maintenance and clothing, or if he does not give her her right to conjugal relations due to an ailment or asceticism.
