Is uttering obscenities—God forbid—while suffering from obsessive-compulsive disorder, without intending to, and with doubt about having uttered them, or making hand gestures that constitute an insult unintentionally, considered as though the speaker has intentionally uttered an obscenity?

Whoever's tongue precedes him, so he utters something unintentionally, then there is no blame upon him, as Allah the Almighty says: "And there is no blame upon you for that in which you have erred, but [there is blame for] what your hearts intended." And whoever doubts whether he spoke or not, the default is that he did not speak. This is not considered an act that leads to disbelief, especially if it originates from someone afflicted with intrusive thoughts (waswas). This is because whoever entered Islam with certainty does not exit it except with certainty. The scholars have said that if there are ninety-nine reasons to declare a Muslim a disbeliever and one reason to keep him within Islam, then one should act upon that one reason.
