Even if the eater knows they will not be able to perform or wudu afterwards due to fear of harm, becomes obligatory for them at that time. This is more deserving of permissibility than intercourse for someone lacking water. The fear of harm must be based on experience or the advice of a skilled physician. If a trustworthy physician advises that ghusl is harmful after consuming the food, then one should resort to tayammum until the fear of harm subsides.
