It is permissible to give to a sick person who needs money for medicine or surgery. They are not obligated to sell their car, which is their source of livelihood and work. The scholars have mentioned that whoever has a craft and needs its tools, should be given from Zakat the price of their craft's tools, even if the amount is large.
