The answer

There is no objection to specializing in surgery or other fields, provided that the limits of Islamic law are observed. However, it is not permissible to uncover the forearms in front of non- men unless there is a necessity. Therefore, if the specialization necessitates uncovering the forearms in front of non-mahram men, then it is not permissible. But if it is possible to maintain the during study, then there is no objection.
