It is permissible for a sick person who finds fasting difficult to break their fast in Ramadan, based on the Almighty's saying: ﴿So whoever among you sights the moon of the month, let him fast it; and whoever is ill or on a journey, then an equal number of other days.﴾ Scholars have divided diabetic patients into four categories. The first and second categories are obligated to break their fast and are not permitted to fast, to avert harm. They must offer expiation (feeding the poor) instead of fasting because their illness is chronic. As for the third and fourth categories, they are not permitted to break their fast because fasting does not harm them, and they must make up for the fast if they break it.
