If you have divorced your wife twice, and taken her back each time within her waiting period, then divorced her a third time while she was pregnant, she is irrevocably divorced from you (Baynunah Kubra). It is not permissible for you to take her back unless she marries another man willingly, he consummates the marriage with her, and then he divorces her or dies, and her waiting period ends. If the divorce was uttered over the phone, it is valid. However, if it was only written without being uttered, it is a metaphorical (kinayah) divorce and does not take effect unless intended.
