Does divorce occur in the aforementioned cases, considering that the wife was provoking the husband to divorce her in the last two instances?

If a husband divorces his wife with three pronouncements of divorce, she becomes irrevocably separated from him (baynunah kubra). He cannot take her back unless she marries another man and he consummates the marriage with her, and then either divorces her or dies, and her iddah (waiting period) expires. The husband's anger at the time of uttering the second and third divorce pronouncements does not prevent their effectiveness as long as he was aware of what he was saying. Similarly, the wife's insistence and threats do not prevent the divorce from taking effect as long as he pronounced it while being aware and of his own free will. If the pronouncement of divorce occurred while he was overwhelmed in his mind or coerced, then his divorce does not take effect. It is advisable to present the matter to trustworthy scholars.
