If the patient can delay taking their medication until after Iftar without harm, they must fast. But if they cannot do so, fearing harm, they are permitted to break their fast and must make up the missed days. If the illness is chronic and there is no hope of recovery, then for each day they miss, they must feed a poor person. Fasting on behalf of a living person is not valid.
