The answer

A pregnant woman is permitted to break her fast in Ramadan if she fears harm to herself or her fetus. She must make up for the days she missed after the excuse is no longer present.

If she is compelled to take medication during the daytime in Ramadan and it is not possible to take it at night, there is no sin upon her for breaking her fast to preserve her health.

She must make up for the days she missed. It is not permissible to delay making up the fast until after the next Ramadan, except for a valid excuse.

If she delays making up the fast without an excuse until the next Ramadan begins, she is obligated to pay a expiation for the delay along with making up the fast. This expiation is feeding one poor person for each day (one "mudd" of food, which is approximately 750 grams of rice). The expiation is not obligatory if she was unaware of the impermissibility of delaying the make-up fast.
