Is the day off, on which attendance and departure are signed, and which is customary in the Ministry of Health, considered lawful or unlawful?

An employee must adhere to the regulations of the employing entity with which they contracted. This includes adhering to attendance and departure policies and work timings, and not being absent, late, or leaving except as permitted by the regulations. If absence with proven attendance and departure is in accordance with the employing entity's system, then there is no objection to it. However, if it violates the system, then it is impermissible. The mere commonness of the practice or permission from an unauthorized official does not make the violation permissible.
