Intentionally breaking the fast during the day in without a valid excuse is a major sin. Breaking the fast due to illness is permissible if fasting becomes difficult, or if one fears the illness will worsen or recovery will be delayed.

Whoever intentionally breaks the fast in Ramadan must repent and make up the ten days. If someone delays making up the seven days without a valid excuse until the next Ramadan begins, they must feed a poor person one mudd of food (approximately 750 grams of rice) for each day. However, if they were unaware of the prohibition of delaying, then there is no expiation for them; they only need to make up the days and frequently seek forgiveness and perform good deeds.
