Does swearing, "By divorce, I will never, ever see or witness anything forbidden, nor will I ever witness it again," with the intention of never witnessing it again, indicate repeated divorce upon repeated breaking of the oath? And does one divorce or more occur upon breaking this oath regarding multiple things simultaneously?

According to the majority of scholars, and this is the most preponderant opinion, a conditional divorce takes effect upon the occurrence of the condition it is tied to, even if the person did not intend divorce. However, Shaykh al-Islam Ibn Taymiyyah holds that if divorce was not intended, then it necessitates an expiation for a broken oath (kaffarat yamin). If someone takes a single oath concerning several things, and then breaks the oath by doing some of what was sworn against, the oath is dissolved, and the divorce does not recur. The expiation for this is a single expiation for a broken oath.
