If the husband is unable to have intercourse due to a narrowing in the wife's private parts, and this narrowing is unnatural, then treatment is advised, based on the Prophet's (peace be upon him) saying: "Seek treatment, for Allah has not sent down a disease but that He has sent down a cure for it, or he said: a medicine, except for one disease. They said, 'O Messenger of Allah, what is it?' He said, 'Old age.'"
As for the ruling: - If the narrowing is natural, and the impediment is the unusually large size of the man's glans, such that it cannot fit any woman, this is considered a defect that gives the woman the option (to annul the marriage). - If the narrowing is in the woman, such that she experiences 'ifdha' (a tear or rupture) from any act of intercourse, then this is a defect in the woman (like 'ratq' - vaginal imperforation), and the husband has the option (to annul the marriage).
A woman with a constricted passage, if she experiences 'ifdha' from any act of intercourse, is likened to a 'ratqa'. And a man with a large organ whose glans cannot fit a woman is analogous to an 'aneen' (impotent man).
