Does a divorce pronounced by someone who then forgot the exact wording he uttered, and does not know whether that wording constitutes a divorce or not, actually take effect?

If a person is certain that he uttered an explicit divorce (talaq sareeh), but then forgot the exact wording or doubted it, his divorce has occurred. However, if he doubts whether he uttered an explicit or non-explicit form, then the divorce does not take effect with doubt.
