What he is feeling might also be the result of psychological pressures, and he should strive to alleviate them and remember his Lord. If the fiancé recovers, then that is good and excellent. But if his condition remains as it is, it is preferable to annul the engagement, because engagement is merely a promise of marriage, and either party has the right to annul it whenever they wish.
