Is it permissible for a wife to deny her husband intimacy or to seek khul' due to his refusal to attend to his personal hygiene and his forsaking of prayer?

The husband should be advised and reminded about cleanliness and prayer. This reminder and guidance should continue if he shows even partial responsiveness, while exercising patience. If he does not respond and the harm from his lack of cleanliness is clear, then the wife may refuse him, or seek divorce, or khul‘ (a form of divorce initiated by the wife); because the husband is compelled to remove anything that causes harm to a person.
