Water must reach every external part of the body during (ritual bath). For visible parts of perforations (in the ear or nose), water must reach them without causing harm. However, for internal or deep parts, it is not obligatory. Al-Nawawi stated: "If one's skin is torn due to a wound... and it is possible to convey water to its visible inner part without harm, then it is obligatory to convey it during ghusl and wudu'."
