And is purification obligatory when one enjoys sexually arousing scenes without the release of ejaculatory fluid, especially when suffering from obsessive-compulsive disorder?

Janabah linguistically is the opposite of proximity, and Islamically it is the major ritual impurity that prevents one from performing acts requiring purity, such as prayer.
