What are the conditions for a complete and accepted intercession, along with the evidence?

Intercession has three conditions: Allah's رضا (pleasure/approval) with the intercessor, His رضا with the one for whom intercession is made, and His permission for the intercessor to intercede, as a confirmation of His, the Almighty's, saying: "And how many an angel is there in the heavens whose intercession will not avail at all except after Allah has permitted [it] to whom He wills and approves." (Quran 53:26), and His saying: "That Day, no intercession will benefit except [that of] one to whom the Most Merciful has given permission and whose word He has approved." (Quran 20:109), and His saying: "Who is it that can intercede with Him except by His permission?" (Quran 2:255).
