Is it permissible for the second wife to request, as a condition for reconciliation, that her husband not come to her on days that are not hers, and that she have his general permission to leave the house, even without his consent? What is the Shar'i ruling in the event of her insistence and the husband's disapproval?

A wife is obligated to obey her husband in matters that do not involve disobedience to Allah. It is forbidden for her to leave his house except with his permission.

Therefore, a wife who leaves her husband's house and stays with her family without his permission is disobedient and rebellious (nashiz), and she is not entitled to maintenance unless she is pregnant, in which case the maintenance is for the pregnancy, not for her.

The husband must be just among his wives, and it is forbidden for him to enter his wife's house on a day not designated for her, except out of necessity. If the spouses agree that the wife may go out at a specific time and the husband consents, it is permissible, provided that the outing is for a permissible purpose, with adherence to full hijab, avoidance of perfume and adornment, refraining from mixing with men, and ensuring safety from temptation (fitna).

If the husband does not consent to his wife consistently going out on a specific day, it is forbidden for her to go out. If she insists, she is sinful and rebellious (nashiz), and she is not entitled to maintenance unless she is pregnant.
