What is the ruling on a man divorcing his wife three times while she is pregnant, denying his intention in the first and second instances, and in the third, saying to her: "You are divorced six times, all at once," in addition to his repeated statement: "If you do such and such, you will be divorced"?

The husband's statement to his wife: "You are divorced six times" is considered an irrevocable divorce (Talaq Bainunah Kubra), according to the most preponderant opinion among scholars.

A clear divorce (talaq sareeh) does not require intention to be valid. However, if the divorce is by way of an implicit statement (kinayah), then it depends on the husband's intention.

Based on this, the wife becomes unlawful for her husband, and he is not permitted to marry her again unless she marries another husband out of genuine desire, and then that husband divorces her.

As for the ruling concerning someone who made his wife's divorce conditional on something, and she then did it out of forgetfulness, you may refer to Fatwa no. 37053.
