These are obsessive whispers that should be ignored and disregarded. The default assumption is that nothing has exited. If one is certain that something has exited, then ablution (istinja') is obligatory, and whatever has stained the clothes must be purified for prayer. As for anything not certainly exited, it holds no weight.
