Regarding a husband's verbal declaration to his wife, "You are divorced by three," a triple divorce occurs according to the majority of scholars. She becomes irrevocably divorced from him, and she is not permissible for him until she marries another man, he consummates the marriage with her, and then he divorces her. The wife's knowledge or lack thereof has no bearing on this. However, if it was merely an internal thought without verbalization, no divorce occurs.
