Is a woman permitted to request a divorce if her husband suffers from severe sexual dysfunction resulting from a short penis, knowing that he experiences weak erections and difficult ejaculation, and there is no available treatment for his rare condition; and what is her ruling if she chooses to remain with him?

The general rule regarding a woman seeking divorce is that it is impermissible or disliked, unless there is a legitimate Sharia reason. This is based on the Prophet, peace and blessings be upon him, who said: "Any woman who asks her husband for divorce without any strong reason, the scent of Paradise will be forbidden to her." However, if she finds harm in remaining with her husband, such as his inability to engage in sexual intercourse, then there is no blame upon her for seeking divorce. The proof for this is the Prophet's, peace and blessings be upon him, approval of the wife of Thabit ibn Qays when she sought divorce from him. And if she chooses to remain and endure the harm, she is permitted to do so.
