Should a mother's feeling of guilt and responsibility for the death of her son—due to not responding to his refusal to go on the trip where the accident occurred—necessitate her blaming herself and alienating the child's father?

We ask Allah to reward you for your calamity and compensate you with something better. What happened is by Allah's decree and destiny, and your suggestion to go with him is not a cause of death, nor does it entail blame, blood money (diyya), or expiation (kaffara). It is incorrect to say that had he stayed, he would not have died, because Allah forbade us from being like those who disbelieved and said to their brothers: "{Had they been with us, they would not have died or been killed}." Everything is by Allah's decree, and even if you had been in your homes, those for whom death was decreed would have come forth to their resting places. Therefore, fear Allah and be patient, and do not let these whispers spoil your life and the life of your family.
