Did the triple divorce occur in this case, and what is the Islamic ruling on what transpired?

If the mufti who issued a single divorce ruling is trustworthy in his knowledge and piety, then there is no blame upon them for following his ruling, according to the words of Allah Almighty: "So ask the people of the message if you do not know." (Surat An-Nahl, 16:43). And whoever seeks out people of knowledge and ijtihad (independent reasoning) and believes them to have arrived at the truth, then he is excused, based on the Prophet's (peace be upon him) saying: "Whoever is given a fatwa (religious verdict) that is not sound, then its sin is upon the one who issued it." (Sunan Abi Dawud, 3660; Sunan Ibn Majah, 204). And direct consultation with people of knowledge is better for clarifying the details of the issue.
