Is raising the hands during prayer done with every bowing and rising from it, or only with the bowing of the first rak'ah and rising from it?

It is recommended to raise the hands in prayer at four points: the opening Takbir (Takbirat al-Ihram), bowing (ruku'), rising from bowing, and when standing up for the third rak'ah. This recommendation is repeated in every rak'ah where bowing and rising from it occur.
