The treatment for obsessive thoughts is to ignore them and pay them no mind, along with seeking medical treatment and consulting the counseling department. As for urinary incontinence ( سلس البول), if the patient does not find sufficient time for purification (الطهارة) and prayer, she should perform ablution (الوضوء) after the time for prayer begins and pray with that ablution until the time for prayer ends. If she does find sufficient time, she should wait for that time to pray. She may also adopt the Maliki school of thought for ease in times of hardship. Regarding what you mentioned last, you can consult the counseling department.
