For istinja' with water, washing only the outer part of the excretory opening is sufficient; introducing water inside is not prescribed. Water remaining on the opening of the penis is not considered impure nor does it nullify ablution, as long as it does not enter the urethral passage. However, if water enters inside the penis and then exits, it is considered impure and nullifies ablution. One must be careful not to give in to obsessive thoughts, and it is recommended to sprinkle water on the private parts and trousers to dispel them.
