The question

What is the ruling concerning my previous prayers in which I used to say "Allahu Akbar" (God is Most Great) when rising from bowing, and "Sami'a Allahu liman hamidah, Rabbana wa laka al-hamd" (Allah hears those who praise Him; O our Lord, and to You belongs all praise) after standing up from bowing, due to my ignorance? Do I owe anything? And what is the ruling concerning my prayers in which I used to invoke blessings upon the Prophet in the first tashahhud? And do I have to perform the prostration of forgetfulness if I make a mistake and do that now?

The answer

The prescribed remembrance when rising from bowing (ruku') is tasmee' (saying "Sami'a Allahu liman hamidah" - Allah hears those who praise Him) and tahmeed (saying "Rabbana wa lakal hamd" - Our Lord, to You be all praise). Saying takbeer (Allahu Akbar) when rising from ruku' is not prescribed. Adding it does not invalidate the prayer, nor does it necessitate a prostration of forgetfulness (sujood as-sahw) unless it was done out of forgetfulness.

Praying upon the Prophet, peace and blessings be upon him, in the first is, according to the more correct opinion, neither a pillar nor an obligation nor a recommended act. The prayer is not invalidated by adding it.

The questioner's prayer is not invalidated by adding some prescribed adhkar (remembrances) in places other than their designated spots. She must learn the conditions for the validity of prayer, its pillars, obligations, sunan (recommended acts), and invalidators.

As for her sometimes making mistakes in prayer, the type of mistake must be specified, because not every mistake is rectified by a prostration of forgetfulness, and not every instance of forgetfulness necessitates prostration.
