The answer

' (cleaning oneself after relieving nature) involves washing the exit point only, unless the discharge has spread, in which case one must wash whatever has been soiled by the impurity. Washing the testicles during istinja' is not legislated, but there is no harm in doing so for cleanliness. It is mustahabb (recommended) – but not (obligatory) – to wash the testicles when madhy (pre-seminal fluid) is discharged. This is the view of the majority of scholars, based on the of Ali (may Allah be pleased with him). Some scholars have deemed this narration weak, while others have authenticated it. Some scholars held the view that it is obligatory, but the majority interpreted it as recommended, or that it applies if the testicles have been stained by some madhy.
