What is the ruling on purification (taharah) and prayer (salat) for a patient with neurogenic bladder whose urine flow is intermittent, and from whom wadī, madhī, and manī are discharged upon istinjā’ (cleaning oneself after urinating or defecating), and who suffers from obsessive-compulsive disorder regarding urination? Does he perform wudu’ for every prayer? And is his purification sufficient for only one obligatory prayer?

If the sensation of urine exiting is merely an illusion and a whisper [from Satan], then it should be ignored. However, if it is a firm certainty, then the situation is one of two cases:

1. If the questioner does not find sufficient time for purification and prayer, then she is afflicted with incontinence, and she must perform ablution after the entry of the prayer time, and protect herself by tying a cloth over the area, and pray with her ablution as much as she wishes until the time exits.

2. If the urination stops for a time sufficient for purification and prayer, then she must protect herself by tying a cloth and wait for this time to perform ablution and pray.

Maliki jurists make things easy concerning the impurity that exits in this situation.

And if she doubts whether what exited is semen or prostatic fluid, then she chooses and gives it the ruling of one of them.
