What should a young man do who suffers from a weak heart and poor financial circumstances, while experiencing intense desire and wishing to avoid the forbidden?

Heart diseases are many. Some of them permit marriage if the specialist doctor approves it. The affected person must disclose their illness, otherwise, they would be deceitful.

However, if the doctor determines that marriage poses a risk to the patient's life, then they are not permitted to marry, as it involves casting oneself into destruction. They must be patient and seek help through fasting. If they are unable to fast and fear falling into sin, masturbation is permissible for them as it entails committing the lesser of two harms.

Shaykh al-Islam Ibn Taymiyyah holds the view that masturbation is forbidden according to most scholars, but it is permissible in cases of necessity, such as fear of fornication or illness. Groups of early and later scholars have permitted it. It is mentioned in "Sharh al-Muntaha" that masturbation is forbidden unless it is done out of fear of fornication, sodomy, or fear for one's body. The condition for its permissibility is the safety from harm, based on Allah's saying: (And do not throw [yourselves] with your [own] hands into destruction) and the Prophet's saying, peace be upon him: (There should be neither harming nor reciprocating harm).

Before masturbation is permissible, if it is possible to take medications that reduce desire without harm, that option takes precedence over masturbation.
