No blame is attached to nocturnal emission (wet dream), as it is a normal occurrence. Ghusl (ritual bath) becomes obligatory if a woman sees semen. To treat fantasies, one should fill free time with what benefits them in religious and worldly matters, strive in du'a (supplication), engage in dhikr (remembrance of Allah), ponder over His names and attributes, and reflect on the Hereafter. Regarding discharges: if it is semen (maniyy), ghusl becomes obligatory. If it is pre-seminal fluid (madhiy), wudu' (ablution) becomes obligatory.
