Does a divorce take effect on the wife if her husband says to her: "I heard her divorced" or "You are divorced," and then claims that he said "Tayyib (okay)"?

If a woman claims that her husband divorced her, but he denies it, then his word is to be accepted because the default is the continuation of the marriage. If the husband explicitly states that he uttered the word "tayyib" (okay) and not "taliq" (divorced) or "anti taliq" (you are divorced), then nothing is binding upon him. Even if the wife is certain that he uttered the divorce pronouncement, it is more likely to be considered a slip of the tongue, and divorce does not occur due to a slip of the tongue. Consequently, the divorce has not taken place, and the wife remains under the marital bond of her husband. There is no sin upon her for not informing him of what she heard.
