Is a husband obligated to treat his sick wife, even if the illness is chronic like insanity or paralysis, or does her treatment fall upon her family?

The majority of scholars hold that a husband is not obligated to provide medical treatment for his wife, and the cost of her treatment is not included in the obligatory maintenance (nafaqa). This is the relied-upon view of the four madhhabs. However, some Malikis are of the opinion that a husband is obligated to provide for his wife's medical treatment. As for charitable giving (تبرع), a husband should treat his wife if he is able to do so and she has no money, due to the kindness and good companionship inherent in such an act.
