Is it necessary for the answer of one who wants to stay alive to be that he wants to stay alive to worship Allah, and is it considered minor or major Shirk if the answer is different, knowing that he will continue his acts of worship?

The primary purpose behind the creation of humankind is the worship of God alone. If a Muslim comprehends this truth, they will become preoccupied with obeying and worshipping God, prioritizing God's will above all else. The answer to the aforementioned question, if it does not involve the worship of other than God, is not considered shirk (polytheism); however, it should be corrected for the one who poses it. Moreover, one must be wary of obsessive whispers (waswas), especially in this domain, because indulging in them leads to great evil.
