What is the ruling on ghusl and prayer if the hands were not washed with soap after istinja’ and the body was not covered with water three times, right and left? And what are the fard (obligatory) acts of ghusl?

It is not a condition to wash the hands with soap after performing istinja' (washing after defecation or urination). It is sufficient to wash them with water until the visible impurity and its smell are removed. If it is difficult to remove the smell, this does not prevent the purification of the area, as the removal of color and smell in case of difficulty is not a condition for purifying the area. Washing three times is recommended, but omitting it does not affect the validity of the purification.
