What is the ruling on one who testifies that there is no god but Allah and that Muhammad is the Messenger of Allah, while he drinks alcohol and eats in Ramadan?

Drinking alcohol and breaking the fast in Ramadan without a valid excuse are major sins. Whoever does this considering it permissible or out of denial is a disbeliever. Otherwise, he is a sinful Muslim facing grave danger.
