How are the Mu'awwidhat to be repeated morning and evening? Is each Surah to be recited three times individually, or are the three Surahs to be recited together and considered one instance, then repeated twice more?

It is narrated in the Hadith of Abdullah ibn Khubayb that the Prophet (peace be upon him) said: "Say: 'Qul Huwa Allahu Ahad' (Surah Al-Ikhlas) and the two 'Mu'awwidhatayn' (Surahs Al-Falaq and An-Nas) three times in the evening and in the morning; they will suffice you against everything."

This Hadith guides us to recite these Surahs three times, without restricting it to a specific manner. The principle for an absolute text is to act upon it absolutely.

Therefore, it is permissible to recite them collectively (Al-Ikhlas, then Al-Falaq, then An-Nas) three consecutive times, or to recite each Surah individually three times (Al-Ikhlas three times, then Al-Falaq three times, then An-Nas three times). Both methods fulfill the requirement of reciting them three times.

This is similar to the scholarly difference of opinion regarding the preferred method of saying Tasbih, Tahmid, and Takbir after prayer—whether it is better to combine them or to say them individually, while both forms are permissible.
