What is the ruling on each of the following: someone who consistently prays at home while acknowledging its obligation in the mosque, someone who consistently prays but delays Fajr until sunrise (Ishraq), someone who consistently prays the five daily prayers but delayed Asr out of laziness and prayed it with Maghrib, and someone who consistently prays all prayers but once left Asr out of laziness? Is any of them considered an apostate (kafir), disobedient (asi), or a perpetrator of a major sin (murtakib kabira)?

Not performing congregational prayer is not considered disbelief (kufr) by the consensus of scholars; rather, the obligation of congregational prayer is a matter of disagreement among jurists. As for abandoning an obligatory prayer due to laziness, scholars have differed on whether it constitutes disbelief or not, with the majority holding that it does not. And according to the view that abandoning prayer is disbelief, if someone misses one prayer or delays it beyond its prescribed time and then repents, they are not considered a disbeliever.
