Is a person afflicted with severe obsessive-compulsive disorder and psychological and behavioral disturbances held accountable for actions and words involuntarily uttered by him? Does he sin by the lukewarmness of his worship or by making wrong decisions due to his condition? Is he permitted to isolate himself from his family to avoid harming them?

You are not held accountable for the quick anger, tension, and agitation that occur against your will, due to the Almighty's saying: "And there is no blame upon you for that in which you have erred, but [only for] what your hearts intended," and the Prophet's (peace and blessings be upon him) saying: "Indeed, Allah has pardoned for my nation their mistakes, their forgetfulness, and what they are coerced into." Furthermore, you are not sinful if you think about leaving your family, but we do not advise you to do so because it may worsen your psychological state. You should strive for treatment and take lawful means, and make abundant supplication, for there is no disease but it has a cure, and Allah answers supplication.
