It is necessary to declare the intention when commencing the make-up fasts for Ramadan (definition), by intending to make up for the missed days of Ramadan. This intention includes days for which a fidyah (definition) (expiatory payment) was due or not. It is not necessary to utter this intention verbally. As for the fidyah, there is no necessary link between it and the make-up fast. It is permissible to pay it along with the make-up fast, which is preferable, or after it.
