What is the ruling on marriage for someone who transitioned from female to male seven years ago without a fatwa, knowing that he possesses masculine characteristics apart from the artificial male organ, and has previous medical reports proving high male hormones and a tendency towards masculinity? And do these reasons permit his transition?

It is not permissible to perform sex-reassignment surgeries, as they involve altering Allah's creation, unless specialized doctors determine that the apparent external genitalia do not match the internal reproductive organs. As for the marriage of a person whose sexual identity is undetermined, this is closer to the case of an ambiguous intersex individual (khuntha mushkil). Many scholars have held that it is not permissible for such a person to marry either gender. Regarding the issue of penis transplantation, this is a contemporary matter. It is more likely that donating an organ from a living person, or from a deceased person based on the traditional concept of death which considers life to persist as long as the heart beats, is not permissible.
